Bispebjerg Hospital and the patient’s own homes
Adult patients (≥ 18 years) admitted with an acute medical disease and who are scheduled for discharge to their own homes within five days from inclusion.
Included patients are monitored with WARD equipment during the last part of hospitalization and the first days after discahrge. Monitoring will last for a maximum of eight days.
